I've lectured about this. It's an underrecognized issue, at least in the US. When you look at rates of PTSD by profession worldwide, train conductors are surprisingly high on the list. There's the accidents, but suicide involving trains or rail is fairly high as well — in the US this is masked a bit by access to firearms and other lethal products, which tend to be more restricted worldwide.
